I hereby apply for a position as stated above. As a condition of my volunteering, I give permission to Malibu Little League (MLL) (for which I’m applying), the right to conduct a background check on me, which may include a review of criminal and/or child-abuse records maintained by governmental agencies as required by Little League and the MLL Safety Plan. In addition, I have read and agree to the Acknowledgement of Responsibility Statement provided on page two. I understand that violations of my responsibilities may result in disciplinary action, as well as action by the Board of Directors, including (but not limited to) suspension and/or expulsion from MLL as specified in the By-Laws of the League and/or Little League International. I understand it is my responsibility to educate and encourage positive attitudes and behavior and will attend all mandatory Coaching Workshops. Applicant’s Signature

Malibu Little League Baseball Acknowledgement of Responsibility I understand and agree that I, as a representative of Malibu Little League (MLL) and Little League International, do not have a license to do as I please as I carry out my duties with my team. I am aware that I am working with the youth of our community and that restraints on my actions may be necessary. I also understand and agree that my choice to volunteer as a manager or coach of a recreational youth sports team comes with the additional responsibility to be mindful of my actions and in control of my emotions at all times. I accept that my actions set a standard for the team and the parents and that I hereby agree to conduct myself in a manner that exemplifies the ideals set forth by MLL and its governing body. I also understand that if I am approved as a manager, I may recommend other people to assist me (assistant coaches, team parent, scorekeeper, etc.) and that all individuals nominated for these positions must be approved by the Board of Directors. I understand that I am responsible for their actions as well. I am aware of MLL’s and Little League International’s principles and upon approval as manager, I will be given the Little League Rulebook, Local Rules, and Safety Manual that outline these principles. I will act responsibly and according to those principles and take responsibility for not only my actions, but also the actions of my assistant coaches. I am aware that game rules are the Little League Official Rules, which may or may not have been superseded by the Local Rules or Safety Manual. I understand that the umpire is in charge of the field and that his/her ruling may not be excessively challenged during the game. If I feel that a decision is incorrect, I can protest the game and have the ruling reviewed by the MLL Rules Committee as outlined by the Little League Rulebook. I understand that excessive challenges are grounds for the umpire to eject me from the game. I also understand that if I am approved to manage or coach a team, by taking on that responsibility, I agree to enforce the rules and directives of the Malibu Little League board of directors and that failure to follow the rules or directives can cause my immediate suspension and/or termination as a manager or coach from the League. I understand that using crude, abusive, or obscene language or gestures toward players, umpires, managers, coaches, and/or spectators is strictly forbidden. I understand that part of my responsibility as a League representative is to discourage this conduct at all times. I understand that corporal punishment is illegal and that MLL is obligated by law to notify local authorities if it is used. I understand that any disciplinary action I intend to use must first be approved by the League President, Vice President, Division Commissioner, and/or the MLL Board of Directors depending on the severity of the problem. I understand that if I am suspending a player from a game because of conduct that I must notify the League President, Vice President, Division Commissioner and/or the MLL Board of Directors, if necessary, before suspending the player. I understand that the use of tobacco products is prohibited (per City/School District Ordinances) on City/School grounds or in the playing area and dugout. Because our games are played on government and/or City property, I further acknowledge that alcohol and drugs are strictly prohibited, and I may be called upon to assist in removing spectators engaged in the consumption of or under the influence of these products. If there is evidence that I have used these products before a game or practice, I may be asked to leave the field or be subjected to further disciplinary action. Finally, I understand that as a representative of MLL and Little League International, I must be legally licensed to drive and possess adequate liability insurance, as mandated by state law, while transporting children other than my own to and from games or practices. I will not allow others to transport such children if I am aware that they do not comply with this requirement.
