Athletic Transcript Evaluation Request Form
NCAA TRANSFER ELIGIBILITY STANDARDS QUICK REFERENCE SHEET KNOW THE RULES: In order for transfer student athletes to be eligible to compete, their transcripts must be officially evaluated by an MSUB Academic Advisor and certified by the Athletics NCAA Compliance Officer. Additionally, two and four-year college transfer student-athletes who have not previously enrolled full time at an NCAA Division I or II institution prior to August 1, 2007, must be certified in amateurism prior to competing in intercollegiate athletics. What Do I Need To Do? Fill out the Transcript Evaluation Request Form attached.
Be sure to attach (unofficial) copies of transcripts from EACH institution you attended. (We need transcripts from each institution as we may accept courses differently than the previous institution.)
Send completed form and transcript(s) to Rebekah Gasner, MSUB Compliance Coordinator. Once all documents are received, they will be sent over to the MSUB Advising Center. Email: [email protected] Fax: 406-657-2919
Register with the NCAA Eligibility Center to complete the amateurism questionnaire only (you DO NOT need to send high school transcripts or test scores to the NCAA): http://eligibilitycenter.org. If you had registered with the Eligibility Center (or NCAA Clearinghouse) previously you will not need to register again. Simply log on to the system with the ID and pin issued to you when you registered. If you do not have your ID and/or pin call the Eligibility Center at 1-877-622-2321 to obtain your information.
Complete amateurism questionnaire and sign the final authorization signature online on or after April 1 if you are expecting to enroll in college in the fall semester. (If you are expecting to enroll for spring semester, sign the final authorization signature on or after October 1 of the year prior to enrollment.) NOTE on amateurism: o o
If you are transferring from a NCAA Division I or II school and have already received a final certification in amateurism from the Eligibility Center, you DO NOT need to fill out the amateurism questionnaire on the Eligibility Center website. You must sign the 10.1 Ethical Conduct Statement electronically to request a final amateurism certification.
Academic Eligibility: You MUST pass a minimum of 9 semester or 8 quarter hours during your last full time semester.
You must have satisfactorily complete an average of at least 12 semester hours of transferable credits acceptable toward any baccalaureate degree program per full-time semester (or quarter) of attendance. Of those credits, only 2 physical education activity credits can be counted.
If you are transferring from a 2 year college: you must have a minimum of 6 semester hours of transferrable English credits, 3 semester hours of transferrable math credits, and 3 semester hours of transferable natural or physical science credits.
You must be in good academic standing with a 2.20 transferable GPA.
