AYSO REGISTRATION AGREEMENT FORM Please return this signed form with your Registration Fees AYSO is a VOLUNTEER ORGANIZATION. We are one of the largest youth group organizations in the USA and we depend on volunteers to conduct the program. Without sufficient Coaches, Referees, Division Directors and numerous other volunteers, we would not be able to have enough teams for all the players who register. Therefore, only children of volunteer coaches and referees will be guaranteed placement. Last year we had 130 teams and 70 U-5 players. All children are placed on teams according to age as of August 1. Children must be between the ages of 4 and 19 years of age to participate in our region. Registration fees are as follows: Age Group

After May 29, 2015, REGISTRATION FEES ARE $120. In addition, after May 29, 2015, YOUR CHILD WILL BE PLACED ON A WAITING LIST!! PLAYERS WILL BE ACCEPTED IN ORDER OF DATE REGISTERED. PLAYERS WILL BE PLACED ON TEAMS BASED ON NUMBER OF VOLUNTEER COACHES. You must mail in a signed Lompoc AYSO registration agreement form, a signed parent/athlete concussion awareness form, the completed on-line registration, and proof of payment to Lompoc AYSO at PO Box 1211, Lompoc CA 93438 OR bring to Walk-in Registration on May 9, 2015 from 10am-5pm at the Dick Dewees Center, 1120 W Ocean Ave. If the player has never played with Lompoc Ayso before, proof of Birth must be provided. Please note, your child will NOT be “officially” registered UNTIL we receive your original signed forms at walk-in registration or by mail. Reminder: Anyone who has outstanding bounced checks will not be able register until all debts are paid off. A $20.00 service fee will be charged for each returned check! No money will be accepted through the mail for registration!

Our region is committed to the AYSO philosophy of balanced teams. Balancing will apply to players ages eight and older and involves distributing players on teams according to their age and number of years played in order to equalize team strengths. Every player is assured to play 3/4 of the game during league play and Tournament play, unless the amount of players on the team does not permit or for disciplinary actions which the Division Director, the Regional Commissioner & the Coach Administrator must approve. If this occurs, 1/2 game rule applies. There are no boundaries for Lompoc AYSO Teams – your child may be placed on a team anywhere within the Lompoc Valley (Mission Hills, Lompoc City Limits, Vandenberg Village, etc…) Points of Contact Please refer to our Website WWW.LOMPOCAYSO.ORG for all questions and email the proper persons with your questions. Please contact your Division Director under your proper age group when possible as these directors are your liaison between the board and coaches/parents. I understand that request of specific coaches or teammates will NOT be accepted. Only the head coach is guaranteed to have his or her child(ren) on their own team. I understand the head coach sets the days, times, and location of practices, and that my child is not guaranteed a certain time or place. I will not be notified of the practice schedules until the season begins in August. (So don’t panic). Notification can occur as late as one week prior to the first game. I will commit my child to ONE Saturday game every week for the entire soccer season unless they are in Tournament play. I will arrange transportation to and from games in a timely manner. I will notify my coach if my child cannot make a practice or a game as soon as possible. I understand that if my child misses more than 5 games, (coach’s discretion to ask for a doctor’s note) or in the event of a family emergency, that they may NOT BE ELIGIBLE FOR ALL STARS. I UNDERSTAND THAT NO TRANSFER OR SWITCHING OF TEAMS SHALL BE PERMITTED. I understand that my child is placed in a division according to National Regulations, which is based on his/her birth date. Moving up a division is at the discretion of the Regional Commissioner and/or the Registrar. I understand if my child is moved to an upper division, that child will remain in that division throughout the entire season of play, this will include All-Star Season. I understand SPACE IS LIMITED based on the number of volunteer coaches and referees. I understand ALL coaches and assistant coaches must be Safe Haven Certified AND Coach Certified for the appropriate Division. I understand payment must be received and cleared by our bank prior my child being placed on a team or to receive a refund. I understand the Under 10, 12, and 14 divisions; will have an End of Season Tournament, which my child will participate in, based on accrual of team points. These divisions will play more than 8 games, weather permitting. I understand it is my responsibility to help my child(ren’s) team(s) in the U10/U12/U14 Divisions obtain 100 volunteer points, to be able to participate in the End of Season Tournament. I understand the U6 and U8 division teams will play 8-10 games. I understand U16/U19 teams travel outside the City for games with locations TBD. I understand U16/U19 teams will play on Wednesday, Saturday, or Sunday. Birth certificate will be required for ALL NEW PLAYERS OR players who did not play last season!

AYSO REFUND POLICY Our 2015 season begins in AUGUST with games starting in September ending in November. Refund policy: Refund request must be made in writing prior to JUNE 30, 2015. NO EXCEPTIONS Registration fee may be refunded at the discretion of the regional board, for the following reasons only: * The region is unable to place a child on a team. * Relocation (Moving out of town) of a child prior to June 30, 2015.
(A $25.00 administrative fee will be deducted from each refund.) The parent or guardian must direct the request of refund in writing. A copy or your receipt, registration form, child name and division and the reason and proof for the refund to the Regional Commissioner prior to JUNE 30, 2015, mail to: Lesa Keasler, Registrar; PO Box 1211 Lompoc, CA 93438. No refund will be given because player has just changed their mind and does not want to play after all!!! By registering your child and signing this form, it is assumed you have read, understand and agree to all of the Registration Agreement Terms.
