MEADOWCREEK HIGH SCHOOL AFTER SCHOOL ACADEMIC SUPPORT TRANSPORTATION REQUEST 2015-2016 Through SIG funds, Meadowcreek High School is able to offer free bus transportation for students at Meadowcreek High School staying after school for academic support. There will ONLY be routes at 5:30 PM. *NOTE: Students will be transported to satellite locations in our cluster not at the regular bus stops. Transportation will be on a first come, first serve basis each week. Once buses reach a 48 student capacity, we will not add any more students. Academic Support transportation begins on September 14, 2015 and will conclude December 10, 2015. There are no buses on Fridays. A student must be registered no later than Wednesday at 9:00 AM the week before the student is requesting bus transportation. There will be no exceptions. No students can be placed on a bus if requesting after Wednesday of each week.

*If you are a permissive transfer student, bus transportation is NOT available. It is the student’s responsibility to turn in this form to Ms. Ruiz in Parent Center by 9:00 AM on each Wednesday, the week prior to expected transportation.

STUDENT EXPECTATIONS Purpose: The purpose for afterschool academic support is to provide students with an opportunity to have academic enrichment and provide students with additional academic support for their various courses. In addition, students will also have an opportunity to recover credit for courses previously attempted. Expectations: Listed below are the expectations for all students participating in the after school academic support program.      

Put forth effort and bring supplies to assist with work completion Arrive on time and be prepared to be actively engaged in class Adhere to the rules and guidelines outlined in the Meadowcreek HS discipline handbook Adhere to rules and requests provided by the faculty and staff that are present to assist students Complete all assignments that are assigned Adhere to all rules and regulations outlined in the GCPS handbook for student behavior on the bus

TEACHER EXPECTATIONS All teachers and after school sponsors are expected to be actively engaged with all of the students that are participating in the after school program. In addition, all teachers and sponsors participating in the after school learning program are expected to supervise the students at all times during the academic after school student support sessions.
