Registration Checklist Final date for receipt of Registration Forms for Derry teams is 27th March 2013 Final date for receipt of Registration Forms for all other teams is 6th May, 2013 Registration Form: (NB: Players must wear jersey number as listed on Registration Form throughout Féile competition) Please ensure all following details entered on Registration Form: • Club County; • Name of Club in Irish and in English; • Club Colours; • Form signed by all 24 players; • Team Manager’s Name, Mobile number, E-mail address; • Form signed by Club Secretary; • Dated; • Signed by Bord na nÓg Secretary. Entry Fee: • Entry Fee of €150 – cheque made payable to Féile Dhoire. Additional Information Required with Registration for Official Féile Peil na nÓg Doire Programme: • Digital Team Photograph; • Team and Mentors’ names; • Short Club History. This could include when your Club was founded and by whom; Honours or Titles won to date; Previous Féile participation; County players from Club; Any other relevant information. • Derry Clubs – In order to commence work on Féile Programme please forward Players’ squad number and name as soon as possible to Brian Smith (details below). Completed, signed forms must be in on or before 27th March. Information required for Inspection during weekend of Féile: All participating teams will be subject to random inspections during Féile competitions. Please ensure nominated Team Mentor has the following to hand: • Original Birth Certificate for each player; • An individual passport size photograph of each player signed on the back and attached to their birth certificate.
