Please return this form with your payment to the school office before the start of the new term General Terms for After School Club Please contact us if your child will not be attending After School Club on a day when they have been booked in. For the safety of your children it is essential that we know which children should be on the register so that we know who to expect. Please note that payment is required with the booking in advance of your child attending the club so that we can ensure we have adequate staff cover for the number of children attending. For the same reason once your booking is accepted the fee is not refundable for any cancellations. Thank you for your understanding. Also, cheques made out for the After School Club must only be for these activities and should not include payment for anything else. If your child is unable to attend for any reason you must let the Club Leader know as soon as possible by contacting 07958 299951 or the school office on 01491 836341.
Late collection Late collection up to 5.45 pm will incur a £5.00 fine per child and £5.00 for each additional 15 minutes or part thereof.
