Health Assessments and Immunizations The Board of Education recognizes the importance of periodic health assessments as established by State health regulations. The Board of Education adheres to those State laws that pertain to school immunizations and health assessments. The Board shall require that formal health assessments be conducted prior to initial enrollment and in grade 6. The Board may deny continued attendance to any student who fails to obtain the health assessments required under C.G.S. 10-206. Recognizing the need for continuity of health care, the Board encourages the use of the family physician for the required health assessments. If by December 1st, the requirement is not met, the Board of Education may assume scheduling of this assessment by the school physician and the school nurse. A parent or guardian must be in attendance during the health assessment. Parents wishing their children exempted or excused from health assessments for religious reasons must request annually, in writing, such exemption from the Superintendent of Schools. This request must be signed by the parent or guardian. No record of a pupil’s medical assessments shall be open to the public.
