PICTURE DAY INSTRUCTIONS You must fill out the photo form completely in order for your child’s picture to be taken, even if you are not ordering any pictures. Please write legibly and include your e-mail address and phone number so the photographer can contact you with any questions. View samples of our products online, http://laceywoodphotography.com/sports/baseball or see them on display the day of the shoot. Your team’s pictures will be taken on _______________ (date) at ____________________ (time) at __________________________________________ (location). Please arrive 20 minutes before your scheduled photo time and report to the check-in table to ensure that your child is checked in, in line, and ready to be photographed. In order for a team to be photographed, all players must be in line with a completed order form in hand. Players should be in team uniforms with shirts tucked in, hats on, and the glove or bat with which they want to be photographed. If your child is having a buddy picture taken with a friend, sibling or parent, that person must be with your child when your team is checked in. Please follow these instructions: 1. To order pictures for your child, simply select the desired package or a la carte option on the order form. For trading cards, a magazine cover or a bag tag, make sure you fill out the necessary information toward the bottom left of the form. 2. Please make checks payable to Lacey Wood Photography. 3. Get $5.00 off our Best Value Pack or 5% off any other order when you authorize us to use your child’s photos for promotional purposes.
ACTION SHOT PHOTOGRAPHY TERMS Coverage of each participating child will take place during one game only and will not take place over the course of multiple games. Photography dates will be determined on an as-needed basis, and we will inform you via e-mail as far in advance as possible when we will be providing coverage of your child. A minimum of $150 in action shot orders must be received from your child’s team in order for coverage to be provided (though we may be able to cover your child when they play another team if your team does not meet the minimum). Should your child be absent or injured on the day of your team’s coverage, or if the minimum is not met, you will receive a full refund. Digital images that have been purchased as part of your package will be made available via download, and action shot products will be mailed directly to your home. Should you wish to add custom text to your photo book, it must be submitted to Lacey Wood Photography via e-mail no later than June 30, 2018.
