Teams must complete this nomination form. The coach (where a team has one) and captain should sign the form before it is submitted. The full list of eligible players is available at http://www.volleyballireland.com/PlayerProfiles. Men's Premier teams are to vote for male players, Women's Premier teams are to vote for female players. You are not allowed to nominate players from your own team or to vote for a player in more than one position (except for the defence specialist position). A team that does not vote (complete all sections) will result in its players being removed from the All-Star nominee list. Please fill out the form in its entirety and nominate 7 players i.e. do not leave any line blank. To qualify for an All-Star award in May, players will be expected to have played a minimum number of matches during the season.
