2. USAV Medical Release Form 3. $40 Registra/on Fee (unless paid online) 4. Closed Date Sheet -‐ Available online under the tryout checklist
5. Receipt from Fusion VBC online registra/on (if pre-‐registered) -‐ All Players MUST Register online at www.fusionvbc.com
Team Details & Selec9on • Up to 14 teams this season
– U12, U13, U14, U15, U16, U17, U18
• 10-‐11 Players per team (based on offensive system) • Upward age movement is possible based on ability and needs • Downward age movement will not be allowed • Players/Parents will be no/fied if they have made a team • Offer will stand for 4 days • Will know the team/coach/prac/ce /mes at the /me of offer

Program Details -‐ Cost • Club Fees for 2017-‐ 2018 Season – U12 & U13 $ 850 – U14 $ 1,250 -‐ $1,450 – U15-‐U18 $ 1,450 -‐ $1,900
• Pay schedule for club fees (3 Payments) – $700 First Prac/ce – $600 January 1st (U12/U13 -‐ $150) – Remainder February 1st (U15-‐U18/U14) Dues can be paid online (credit card) or via check.
