NOTE: THE FOLLOWING HOLD HARMLESS AGREEMENT MUST BE READ BY PARENT AND/OR LEGAL GUARDIAN & SIGNED PRIOR TO PARTICIPATION IN THE WILDCAT BASEBALL CAMP. The undersigned parent and/or legal guardian, in consideration of the acceptance of their child as a participant in the Westerville South Wildcat Baseball Camp does hereby: (1) Agree to assume any and all risk and liabilities incidental to participation in the Westerville South Wildcat Baseball Camp by such child; (2) Agree to indemnify, defend and hold the Westerville South Wildcat Baseball Camp Coaches and Volunteers harmless from and against any claim, demand and liability for any injury, and damage or loss of personal property; (3) Acknowledge and understand that no medical insurance is maintained by the Westerville South Wildcat Baseball Camp, such insurance being the sole responsibility of each participant; and (4) Agree that prior medical or health problems must be disclosed by them to the Coaches at the Westerville South Wildcat Baseball Camp in writing prior to any child becoming a participant in the camp. REFUND POLICY: (1) 100% if Withdraw Received 30 days prior to Westerville South Wildcat Baseball Camp Start Date; (2) A $25.00 processing fee will be assed for any Withdraw Received up to 14 days prior to start date of the Westerville South Wildcat Baseball Camp; (3) No refunds after that date.
EQUIPMENT: Westerville South Wildcat Baseball Camp WILL PROVIDE Baseballs and Batting Helmets. Participants are expected to arrive with water bottle, Baseball Cleats, Baseball Hat, Baseball Pants, Baseball Glove, Baseball Bat, and Athletic Supporter. NO Shorts!
