EUGENE KIWANIS FOUNDATION SCHOLARSHIP INSTRUCTIONS The Eugene Kiwanis Foundation is offering four college scholarships to high school seniors at South Eugene and Sheldon High Schools (two scholarships per school). Each scholarship will pay the recipient $1,000 for use at a college or university in Oregon. 1.

Fill out the application form, providing all requested information, with your name at the top of each sheet and return everything to the following email address by April 7, 2017: Rich Snider Email: [email protected]

Give out 2 Scholarship Recommendation Sheets (one to an adult outside the school setting who knows you well, and the other to a teacher or coach who knows your work and abilities). Ask the raters to email the completed forms to the above email address. It is your responsibility to make sure that the application is complete including all requested information such as the Scholarship Recommendation Sheets and transcript. RULES AND REGULATIONS Only graduating seniors with a GPA of 3.0 or higher are eligible for the scholarship. Applicants must show good citizenship and academic achievement.

All application material must be emailed to the Kiwanis Foundation at the above address by April 7, 2017.

Each award will be made as a payment to the scholarship recipient in the amount of $1,000. Payment will be made when the Eugene Kiwanis Foundation receives confirmation that the recipient is enrolled full time (e.g. a copy of the tuition billing or receipt showing payment of tuition).

This scholarship program provides one time awards for students enrolling full time at colleges and universities in Oregon. Exceptions can be made at the discretion of the Eugene Kiwanis Foundation in unusual circumstances.

If a student awarded this scholarship decides to enroll at an institution in another state, the funds will be retained for ensuing year's scholarship applicants.

The scholarship awards will be announced before May 15, 2017. The awards will be presented to the recipients at a regular Eugene Kiwanis meeting. Regular meetings are held at noon on the 1st and 3rd Wednesdays of each month at the Eugene Hilton.

7.

The Eugene Kiwanis Foundation reserves the right to make all final decisions.

ATTACH SUPPLEMENTAL SHEETS WHICH PROVIDE THE FOLLOWING: 1.

A description of your high school activities, community activities, academic and athletic honors, and grade point average. 2. A copy of your transcripts 3. A written statement discussing the following. a. Two activities you have particularly enjoyed while in high school b. The two most important things you feel you have gained or learned while attending high school c. The major you plan to pursue in college, explaining your interest in this field and your occupation goals d. Why you feel you deserve this scholarship 4. The two requested recommendation sheets should be emailed directly by the persons making the recommendations to the email address provided.

The Eugene Kiwanis Foundation will review scholarship applications from qualified and deserving high school seniors in selected areas of Eugene. We request that each applicant provide recommendations from a teachers or coach and from an adult outside the school. This form is intended to help you provide us with information about a student applying for our scholarship. Please provide us with a statement about the student identified above. Information about his/her approach to school, involvement in community projects, and overall direction and work habits is requested. We want to know how successful you believe this student will be in pursuing a degree in higher education. Is the student selfmotivated, able to set and achieve personal goals, and dependable? Has this student shown improvement throughout high school? Please respond to these and other attributes that you feel are appropriate.
