Thank you for your interest in serving at camp and for completing this application. All forms will be reviewed and those selected to work at this year’s camp will be notified the first week in June. Please be in prayer for the planning and preparation of this year’s camp. By submitting this form, I acknowledge that I plan to attend Happy Haven Bible Camp from July 29 th to August 3rd, 2018 and am available for the entire duration of camp, Sunday afternoon through Friday evening. Any exceptions must be approved by Joel, for the purpose of having camp run as smoothly as possible and having adequate staff from the time of the campers’ arrival to their departure. Furthermore, I acknowledge that I have read the attached Staff Expectations page, in its entirety, and will agree to abide by these standards and report to the Camp Director any activity from fellow staff members that is contrary to these standards. Staff Expectations Happy Haven Bible Camp is an incredible opportunity for ministering to children and sharing the Gospel in a Christcentered, safe, and fun environment. In order to ensure that the environment is all of these things, we rely on willing camp staff who are currently living a lifestyle that magnifies Christ and who desire to give a week of their summer to care for, witness to, share Christ’s love with children in a camp setting. In becoming a staff member of Happy Haven Bible Camp, all counselors and leaders have taken responsibility for the campers under their care for the duration of Camp. This responsibility is first to the Lord, and also to Camp, campers, and parents. The following is a list of expectations that we have for all those who will be volunteering as staff with HHBC this year. Christ-Centered In order to help create a Christ-centered environment, staff are at all times expected to:  Model an active and healthy relationship with Christ o Seek to love and honor God with behavior and attitude (Mark 12:30) o Have personal devotions, prayer time, and meditation on scripture daily  Respect leadership, fellow staff, and campers  Follow the Bible’s teachings o Love one another (Mark 12:31, John 13:34-35) o Honor God and each other in the way you speak (Eph. 4:29)  Language should be clean and encouraging, not profane or vulgar o Honor God, each other, and yourself by remaining pure sexually and modeling healthy relationships  No public displays of affection-unless married (occasional hugs or handholding are ok) and no immorality  Do not be alone with member of opposite gender unless otherwise approved by leadership  Do not view or encourage viewership of inappropriate sexual material  Spend meaningful time with campers while sharing God’s word and the Gospel, and praying with them o Find out if campers have ever put their faith in Christ and ask for prayer requests Safe In order to help create a safe environment, staff are at all times required to:  Never be alone with a camper behind closed doors or out of sight  Never use or possess inappropriate or illegal substances or items o Alcohol, tobacco, illegal drugs, firearms, large knives, or other dangerous items are prohibited unless otherwise approved by the Camp Director  Dress modestly and appropriately (all clothing up to discretion of camp leadership) o No low cut or bare midriff shirts, no inappropriately short shorts o Modest swimwear that is one-piece (or approved by same-gender camp leadership) o Staff should not be in an inappropriate state of undress in front of campers while in cabins  Change quickly and use discretion, do not expose yourself to a child  Never use inappropriate touch or allow inappropriate touch between staff and/or campers o Never touch or allow the touching of a child in a private area or in a sexual way o Hugs should be short o No removal of clothing (for example: “pantsing”) or contact beneath clothing (for example: “wedgies”) o Do not encourage prolonged carrying of campers, male staff should not carry females in “chicken”  Never cause or allow any physical harm (including whipping with “rat tails” or pinching/twisting any body part)  Obey curfew unless otherwise approved  Immediately report any violation of these rules or any activity that you perceive to be dangerous or detrimental to campers or staff Fun In order to help create a fun environment we asked that staff keep campers their first priority. We ask that you spend the majority of your time with the campers and your cabin, talk to them, participate in activities, stay engaged, sing along with songs, do motions, be a good sport, and have fun! It would be impossible to create an exhaustive list of conduct guidelines, so we ask staff to approach every situation with prayer and seeking to bring glory to God. Children should never be put in a position where they can be or have been harmed spiritually, emotionally, physically, or sexually. The goal is for every child to be with adults who love the Lord, to always feel safe and loved, and to have a week that they will never forget. If there is ever any doubt for how to handle a situation, please do not hesitate to ask another staff member or camp leader.

Act 153 Volunteer Affidavit I attest to the following statements in lieu of obtaining the FBI clearance as allowed by Act 153 of 2014: 1. I am applying for the position of camp staff/counselor, an unpaid, volunteer position with the Happy Haven Bible Camp of Richfield Life Ministries Church, Smith Corner Church, and Tull’s Hill Bible Church. 2. I have been a resident of the Commonwealth of Pennsylvania for the entirety of the ten (10) year period immediately preceding the date of this Affidavit. 3. I am not disqualified from service as a volunteer as a result of a conviction of one or more of the following offenses listed under Title 18 of the Pennsylvania crimes code, or the attempt, solicitation or conspiracy to commit any of the following offenses: a. Criminal homicide (Chapter 25); b. Aggravated assault (Section 2702); c. Stalking (Section 2709.1); d. Kidnapping (Section 2901); e. Unlawful restraint (Section 2902); f. Rape (Section 3121); g. Statutory sexual assault (Section 3122.1); h. Involuntary deviate sexual intercourse (Section 3123); i. Sexual assault (Section 3124.1); j. Aggravated indecent assault (Section 3125); k. Indecent assault (Section 3126); l. Indecent exposure (Section 3127); m. Incest (Section 4302); n. Concealing death of a child (Section 4303); o. Endangering welfare of children (Section 4304); p. Dealing in infant children (Section 4305); q. Felonies related to prostitution (Section 5902 (b)); r. Obscene and other sexual materials/performances (Section 5903(c)); s. Corruption of minors (Section 6301); or t. Sexual abuse of children (Section 6312). 4. I have not been convicted of an offense similar in nature to those offenses listed in Paragraph 3 above under the laws or former laws of the United States or one of its territories or possessions, another state, the District of Columbia, the Commonwealth of Puerto Rico or a foreign nation, or under a former law of this Commonwealth. 5. I am not disqualified from service as a volunteer as a result of a felony conviction for violation of the Controlled Substance, Drug, Device and Cosmetic Act (35 P.S. § 780-101 et seq.) during the five (5) year period immediately preceding the date of this Affidavit. 6. I have not been named as a perpetrator of a founded report in the Pennsylvania Statewide Database for an act of child abuse committed within the five-year period immediately preceding the date of this Affidavit. I hereby affirm that the information in this affidavit is true and correct to the best of my knowledge and belief and that the signature is being made subject to 18 Pa.C.S.A. § 4903 relating to crimes for false sworn statements which is a misdemeanor of the third degree punishable by up to one year imprisonment. _________ Date
