I give permission for my child’s picture to be used on the web site. I assume all risks and hazards, incidental to the conduct of any of the ac vi es, including transporta on to and from such ac vi es. I do further release, absolve, indemnify and hold harmless the Johnsburg Jr. Skyhawks, its organizers, sponsors, directors and supervisors, and any or all of them, in the case of injury to my child. I hereby waive all claims against the organizers, sponsors, directors and supervisors appointed by JJS. I likewise waive, to the extent not covered by liability insurance, any claim against any person transpor ng my child to and from the ac vi es. Emergency Medical Authoriza on: I the parent/guardian of the above named par cipant, hereby give my authoriza on for any emergency medical treatment of the par cipant for any injury resul ng from any ac vity of the JJS including transporta on to or from such ac vity. It is understood that efforts shall be made to contact the undersigned prior to rendering treatment to the injured par cipant. This authoriza on will expire on 12/31/18. Refunds: All par cipants who request, in wri ng, a refund of their applica on fee prior to the second week of full pad’s prac ce for their team will receive a full refund less $25.00 for handling costs. A er the start of the first prac ce there will be no refunds. All refunds are con ngent upon the return, in good condi on, of all equipment. Refunds will be mailed no less than three weeks a er the request for a refund has been received. Equipment: Each par cipant will be loaned the following equipment: A helmet with face mask and chin strap, shoulder pads, hip/thigh/knee pads, game pants, and game belts. The Game Jersey will have the par cipants name on it and therefore be kept by the individual. Players should provide their own shoes, which should be all-purpose football/soccer shoes with molded sole rubber cleats. Metal cleats or removable cleats are not permi ed. Each player: Is expected to clean on a regular basis and return all loaned equipment at the end of the season. Players should no fy their coach immediately if any equipment is damaged or lost during the season, so it may be replaced. Replacement costs will be assessed to each player for lost equipment. The amounts will be as follows: helmet $120, game pants $30, shoulder pads $45, prac ce pants $20, prac ce jersey $10. Total = $ 225.00. Level of Play: Each player will compete with other players of comparable age and weight, by being assigned to a league level through an age/weight formula. It is the posi on of JJS that it is generally best for each player to play at the lowest level, without any significant weight loss, par cularly which occurs in a short term. The decision to a ain a certain playing weight and the responsibility of maintaining that weight belong to the player and parent/guardian. Generally, all players should be below their maximum weight for the level for which they desire to play. This would be by the first day of prac ce. Player's weight will be checked prior to star ng their first prac ce, and before every game throughout the season. TCYFL PARENT / FAMILY MEMBER PLEDGE 1. I pledge to get my player to prac ce and games on me. I understand that it can be embarrassing for my player to be late. I will be on me to pick up my player a er all games and prac ces. This shows respect for the coaches and other players, and it tells my player that they are my top priority. 2. I understand that the top three reasons kids play sports are to have fun, make new friends and learn new skills. I understand that the game is for the kids, and I will encourage my player to have fun and keep sports in its proper perspec ve. I understand that athletes do their best when they are emo onally healthy, so I will be posi ve and suppor ve. 3. I will properly define what it is to be a "Winner" in my conversa ons with my child. Winners are people who make maximum effort, con nue to learn and improve, and do not let mistakes or fear of making mistakes discourage them. I understand that mistakes are an inevitable part of any game and that people learn from their mistakes. I understand that all children are born with different abili es and that the true measure of success is not how my player compares to others, but how they are doing in comparison to their own poten al best self. 4. I pledge to "Honor the Game," I understand the importance of se ng a good example for my player. No ma er what others may do, I will show respect for all involved in the game including coaches, players, opponents, opposing fans, and officials. I understand that officials make mistakes. If an official makes a "bad" call against my team, I will Honor the Game and be silent! 5. I pledge to refrain from yelling out instruc ons to my child. I understand that this is the coach's job. I will limit my comments during the game to encouraging my child and other players for both teams. 6. I pledge to refrain from making nega ve comments about my player's coach in my player's presence. I understand that this plants a nega ve seed in my player's head that can nega vely influence my player's mo va on and overall experience. 7. If I have a problem with my player's coach, I will call the coach to set up an appointment to discuss the problem, and not confront my coach immediately a er a prac ce or game. I will honor this TCYFL Parent/Family Member Pledge in my words and ac ons Signature: __________________________________________________________ Member Community: Johnsburg JOHNSBURG JR SKYHAWKS PLAYERS STANDARDS OF CONDUCT The Johnsburg Jr Skyhawks Football League has established the following Standards of Conduct for all players. Players are expected to conduct themselves in a responsible, respectable manner both on the field and in the community. Incorporated in the standards are the means by which they will be measured and correc ve ac on taken in the event of viola on. A endance at Games, Prac ce, and Weigh-ins: Ÿ Players are expected to a end all scheduled prac ces and games. Ÿ Players missing more than 25% of the prac ces prior to the first game may not be eligible to par cipate in the first league game. Ÿ Players missing more than one of the weekly scheduled prac ces a er the first league game may see limited playing me during the next game. Ÿ Players missing all weekly scheduled prac ces will not par cipate in the next game. Ÿ Players unable to a end prac ce are responsible for no fying one of their coaches prior to prac ce. Ÿ Players are expected to have all required uniform/equipment at every prac ce and game. Ÿ Players are expected to come to prac ce and/or game with a posi ve a tude and a willingness to put forth an effort for their team and coaches. Conduct on and off the field: 1. Cri cizing teammates, league par cipants, officials, coaches and parents. 2. Use of profanity while on the prac ce or game field. 3. Verbal in mida on of teammates and League par cipants. •Ini al viola on will result in a warning and parent no fica on. A second viola on would result in a one game suspension and a third viola on may result in dismissal from the League. 4. Involvement in physical alterca ons (figh ng) with teammates or League par cipants. •Ini al viola on will result in a warning, parent no fica on of the incident, and possible one game suspension. A second offense may result in dismissal from the League. 5. Involvement in community misconduct, i.e. vandalism, the , violence, defacing, etc. •Community misconduct will result in an immediate one-week suspension. A second confirmed incident would result in dismissal from the League. 6. Use or be in possession of tobacco products, alcohol, or illegal drugs. •If a player is charged by the police with use or possession of tobacco, alcohol, or illegal drugs, the individual will be placed on indefinite suspension pending disposi on of the case. If convicted, the individual will be dismissed from the League and will forfeit any fees paid. All viola ons of the Standards of Conduct will be reported to the Jr. Skyhawks Board. Flagrant viola on(s) may result in a more severe penalty being imposed. The final decision will be the responsibility of the Board.
