I have read and agreed to the following Member/Grower Agreement Form: Plum Granny Farm is offering PlumFresh as a membership subscription program for a weekly box of our fresh fruits and vegetables. The program is designed for flexibility for the members – you can let us know if you need to skip a week and payment can be made in advance or on a weekly basis. The cost of a weekly box is $30 (plus $4 delivery if desired). As a PlumFresh member, you are not just purchasing produce; your commitment supports our farm. By regularly getting your box, you help us better plan and manage our growing and harvesting. Partnering with us helps sustain a local farm with a commitment to Organic, responsibly produced and quality food. Your signature acknowledges that you agree to the following terms of Membership: 1) Contact the farm in advance to cancel a box or boxes; 2) pre-pay or pay upon pick-up for the box (deliveries must be prepaid); 3) pick up your box at a predetermined location (see below) or arrange for delivery. If you do not cancel a box by the cut-off time, you will be charged for the box (we’ll be harvesting veggies for you and will have reserved them for you). Additional program guidelines may be added prior to the beginning of the season. You may cancel your membership at any time but if you cancel prior to the first three boxes, you will forfeit your deposit. PICK-UP OPTIONS Please choose your preferred site ⃝ On-Farm at Plum Granny Farm (Wednesdays from 3-6 p.m.) ⃝ Cobblestone Old Salem Farmers Market (Saturdays from 9-noon) DELIVERY The delivery fee is $4 per box. Delivery is available in limited areas (King area, central and north Winston-Salem, other TBD). ⃝ YES! I am interested in delivery! (Wednesdays late afternoon/early evening.)
