St. Luke Catholic Church Confirmation Program 4603 Manitou Drive, San Antonio, Texas 78228 (210) 433-2777 Confirmation 2017-2018 AGREEMENT FOR ALL STUDENTS ENROLLED IN THE CONFIRMATION PROGRAM & THEIR PARENTS 1.

All students must attend classes, required retreats and other events before they are confirmed. Students are only allowed 3 excused absences. (A note must be turned in to the student’s catechist the class date immediately following the absence explaining the reason for the absence.) 2. All students are to arrive to class on time (class starts at 2:15 p.m. on Sundays). A student will be considered tardy after roll has been taken. Note that three tardies will count as one absence. 3. Students and/or their parents are not to ask their Catechists to allow them to leave before class ends. 4. Students should participate in class discussions and come with an open mind and open heart. 5. Students should respect all individuals in the classroom, as well as respect school property – i.e. no writing on school desks, dry erase boards, etc. 6. Students are not to use electronic devices during class time – this includes all cell phones, Ipods, MP3 players, etc. Use of such devices will result in the device being taken from the student and the parent contacted to come and retrieve it. 7. Students should not repeat what is shared confidentially in class. 8. All students and parents should attend Mass every Sunday here at St. Luke’s. Attending Confirmation class is not a substitute for attending Mass. 9. All parents are asked to present copies of the student’s Baptismal and First Eucharist Certificates no later than October 29th, 2017. 10. The student/sponsor form must be turned in by December 10th, 2017 11. Students must complete a Saint Report by February 4th, 2018. This Saint Report must reflect the Christian name the student will take at Confirmation. 12. Students must attend the Confirmation Retreat which will be held at St. Luke Gym on Feb 9-10 13. The LifeTeen Retreat in November does not take place of the Confirmation retreat. 14. All students must complete 20 hours of community service which must be accomplished by completing the group service projects chosen for the Confirmation program. All service hours will be done from the list provided. All service hours must be completed by Feb 25th, 2018 15. Parents are asked to make note of all of the student’s deadlines for the Confirmation program – i.e. Saint Report, Sponsor/Candidate form, & Service Hour forms. Please communicate with the Confirmation Coordinator when foreseeable problems arise that will cause the student to have trouble meeting these deadlines. WE, THE STUDENT AND HIS/HER PARENT OR GUARDIAN, AGREE THAT WE HAVE READ THE ABOVE REQUIREMENTS FOR THE 2016-2017 CONFIRMATION PROGRAM. Signed: ____________
