AAMC Employee FMLA/Medical Leave Navigation Tool

Employee requests leave (FMLA, Medical Leave of Absence, ADAAA)

Employee contacts The Hartford* 1-877-822-3183 *Administrator of the AAMC Employee Benefit Plan

The Hartford sends medical certification paperwork to employee’s home. Employee takes paperwork to the appropriate physician to complete and return to The Hartford within 15 CALENDAR days.

FMLA claim denied for late or insufficient paperwork. The Hartford notifies employee by letter.

Employee returns to appropriate physician to complete paperwork and submits to The Hartford.

The Hartford issues letter to employee to notify claim determination

FMLA request approved

Employee maintains communication with supervisor/manager, providing advance notice of absences* and planned return to work date. *When calling out, employee should specify any FMLA-related absence.

Employees out of work for 3 or more days for their own health condition must report to the Employee Health Office (EHO) for a RTW slip, prior to returning to their department

FMLA: Family Medical Leave Act ADAAA: Americans with Disability Act Amendments Act EHO: Employee Health Office RTW: Return to Work

EHO forwards the RTW slip to the supervisor, The Hartford, and Human Resources.

FMLA request denied; employee is ineligible for FMLA due to length of service with AAMC and/or does not meet minimum hours requirement.

The Hartford grants AAMC Medical Leave (6 weeks job protection)
