AC SANDHILLS 2015/16 Volleyball Acceptance Agreement By accepting a position with an AC Sandhills Volleyball Team, I agree to be committed to AC Sandhills for the 2015/16 travel volleyball season. I understand that it is a full year commitment to the team, and my daughter is committed to playing for AC Sandhills for his/her entire 2015/16 volleyball season. I agree to meet any and all financial obligations/responsibilities for my player to play for their AC Sandhills team. I agree to make all payments according to the payment plan selected. If I do not make any and all payments on the date due, my child will not be eligible to participate in team training, league games and tournament games. *AC Sandhills 2015/16 Volleyball Fees:

USAV Registration: Every player must be registered with the USAV. This may be done online and costs are $45.00 for players ages 12 to 18, when registered before November 1st. After November 1st it is $55.00 for all players ages 12 to 18. Youth players ages 11 and younger are $15. This is not covered by the club and is required for all participating players. 2015/16 AC SANDHILLS Payment Options: Plan 1: Monthly installments starting Dec. 1, 2015 and ending on March 1, 2016 Plan 2: One (1) Payment at Commitment Night.
Payment Plan 1: (All numbers are in $) Monthly installments starting December 1, 2015 and ending on March 1, 2016 All payments will be due on the 1st of each month. If AC Sandhills has not received payment by the 15th, we reserve the right to charge the given credit card account.

Payments are due on all dates provided in the payment plan breakdowns. There will be a fifteen (5) day “grace” period allowed by AC Sandhills for checks. If a payment is not received after that time, AC Sandhills reserves the right to charge the amount owed to the following credit card given.

By signing this agreement, I have read and understand all of the available AC Sandhills payment options. I agree to pay all of my fees on time and in full based on the payment plan I choose below. I agree and give AC Sandhills the right to charge my credit card account I have willingly provided below in the case that I have become default in making my players payments.
I understand that the AC Sandhills Volleyball Season is a full year commitment, and by signing this document therefore assure
AC Sandhills that I will pay my dues in full regardless of circumstance. If for any reason I have to pull my daughter from the AC Sandhills Volleyball Program due to relocation or financial reasons ONLY, before or during the 2015/16 Volleyball Season, I understand that AC Sandhills will retain a $300.00 administration fee to provide for the loss of team income due to my leaving. If fees already paid do not amount to $300.00 due to my selected payment plan, I will pay the remaining amount up to $300.00 in order to be released from my club financial responsibilities upon agreement by AC Sandhills Executive Director and Board of Directors. I agree that if my daughter is found in default of their fees, that they will not be able to participate in any club activities. (includes training, academies, league games, friendlies, tournament games, etc…)

There will be no further use, unless verified by the card holder, then charging fees upon the stipulations of this document. If AC Sandhills or any affiliated organization illegally charges, transfers or deducts money from your credit card or accounts without the consent of the above card holder, then legal action will be recommended to the card holder as well as taken by AC Sandhills against the violator in question.
