PAGE 40, choose your program, print and sign name. PAGE 41, “Advisory Agreement”; please fill in appropriate info, as well as discussed management fee percentages. PAGE 44, sign and print name, date and address. PAGE 45, “Fee Payment Authorization”; we will fill in the appropriate BROKERAGE FIRM, please sign and print name, date and address. PAGE 46, “Limited Trading Authorization and Power of Attorney”; we will fill in the appropriate BROKERAGE FIRM, please sign and print name, date and address. There will also be a POA (Power of Attorney) form from the trade house your funding is held with that we will ask you to sign, if you haven’t already done so. Please use AFB LLC as the trading agent.
