JAPAN Associate Leadership Renewal Order Form Instructions (*please be sure to fill out the Japanese form) This form is for Mannatech Japan Associates who wish to renew their All-Star Leadership status. Please remember to fill out the original Japanese form, and not the English explanation areas. (If you need a new blank form, you can download the form as a PDF file from the Mannatech Community Resource page, by selecting Japan as the country, and clicking document 1806903) PLEASE REMEMBER WE DO NOT SHIP INTERNATIONALLY.

PAGE 1 (this page) is the instructions on how to fill out the Leadership Renewal Order Form PAGE 2 is a sample English translation. PAGE 3 is a blank Japanese form for you to fill out. ① Please fill in the information for the Associate account under which the order will be placed. Please fill in the date using the 20__MMDD format. ② Please fill out where you would like the products shipped to. ③ Please select a method of payment 1) Cash may only be selected for Shinagawa storefront orders or storefront pickup. 2) For wire transfer payments, a minimum of 24 hours is needed for payment verification. Postal transfers may take longer. (*Sorry, we do not accept international wire transfers.) *Please remember we need an official affidavit if the associate and the credit card name are not the same pperson.
