Athletic Training ACADEMIC YEAR 2017-2018 Dear Parents and Student-Athlete: We are extremely pleased to have your son or daughter as a student-athlete at Appalachian State University (ASU) for the upcoming year. Included in this packet are the Health History Questionnaire and other paperwork required for your student-athlete to obtain medical clearance to participate in athletics at ASU. Please assist your student-athlete in filling out these forms to the best of his/her ability. In order to ensure a smooth process through the pre-participation physical exam process, please have this paperwork filled out prior to coming to campus. It is important for parents to retain this letter and all other documentation provided by the ASU Athletics Department throughout the year as it explains our policies regarding the health and safety of your student-athlete. Physicals: Each new student-athlete is required to have a general medical physical examination by a physician within 3 months prior to participation in intercollegiate athletics at ASU. Our form must be used. If your student-athlete is not able to get a physical prior to arriving on campus, one will be provided. If an orthopedic evaluation is necessary, one will be provided by our team physician once the student-athlete arrives to campus. Note that your student-athlete may need further evaluation apon arriving at App State. The final decision on physical qualifications or reason for rejection is the responsibility of the App State team physician. The decision on when an athlete may return to physical participation following an injury is also the team physician’s responsibility. Sickle Cell Trait Testing: It is Appalachian State University Athletic policy that every student-athlete has proof of Sickle Cell Trait testing and results. We encourage all student-athletes to obtain this test result from their family physician or get the test done prior to arriving in campus and include with returned paperwork. It will be recommended for the student-athlete to get the test performed as part of their pre-participation physical if he/she does not provide record for the test and result.
