Step 3: Print out a copy of the evaluation form

Step 4: Complete the front page of the form yourself, BEFORE you meet with your physician

Step 5: Bring all 3 pages of the Pre-Participation Evaluation Form to your physician

Step 6: Review page 1 of the Pre-Participation Evaluation Form to your physician prior to the exam

Step 7: Have the physician complete page 2 of the PreParticipation Evaluation Form to your physician **Form MUST be signed by an MD or DO**

Step 8: Scan pages 1 and 2

**Save document as F17 Physical**

Step 9: Click Choose File

**Another screen will appear**

Step 10: Select F17 Physical from your computer

Step 11: Type F17 Physical in the Description box

Step 12: Click Upload

If you do not have access to a scanner you may mail a copy of your completed physical form to Aurora University Athletic Training 347 S. Gladstone Ave Aurora, IL 60506
