Authorized Dealers of KODAK Dental Systems and COOK-WAITE Drug Products American Dental Association (October 18-20, 2012) Greater New York Dental Meeting (November 25-28, 2012) We are pleased to announce our ADA and GNY show specials for customers in attendance at the tradeshow below. Our reminder to you regarding the show special fulfillment process. 1. Eligible participants for our tradeshow specials include only those customers in attendance and onsite at the tradeshow. 2. Orders taken at our booth will be processed through the dealer specified by the customer. The dealer does not need to be in attendance at the show. 3. Orders that take advantage of show specials must be placed during show dates. 4. Dealers are responsible for fulfillment of all free goods. Completed claim forms must be received by: ADA – November 30, 2012 / GNY – December 29, 2012. Attachment A–Tradeshow Requirements A list of required information has been provided to ensure prompt and accurate processing of promotional reimbursements including: • Summary sheet is required to ensure immediate processing of your reimbursements. Attachment B – Tradeshow submission form for ADA and GNY • Dealer name • Dealer invoice number • Date of invoice • Practitioner name • Practitioner address • Practitioner phone number • Product description • Units sold • Promotional units – free box(es) Computer reports are acceptable as documentation, provided all the necessary information (practitioner name, practitioner address, and purchase date) is included. We reserve the right to audit dealer’s books and records relative to compliance with the terms of this promotion. Failure to submit promotional documentation per these guidelines will delay the processing of your promotional fulfillment.
