*APPOINTMENTS ARE REQUIRED—Call: (262) 767-7160 Early scheduling is suggested as appointment spots are limited. No same day appointments will be taken.
COST: A $15 fee will be charged for each physical and only apply to the dates and times above. All fees are donated back to the Athletic Department of Badger High School.
FORMS: Health History Exam and WIAA Clearance forms can be obtained from the Badger High School Athletic Office or online at:
www.Badger.k12.wi.us badger/athletics/sport information Completion of the history section of the form is necessary prior to arriving for the physical. The Badger Athletic Office will be requiring online registration for all athletic forms beginning next school year (2016-2017). Additional information will be mailed this summer regarding how to upload the athlete’s physical form and the Consent/Code of Conduct form. Keep your completed physical until you receive additional information from our office.
PLEASE NOTE: No insurance will be taken. No immunizations will be given. This is for high school students only and does not apply to incoming freshman or middle school athletes. Aurora Sports Medicine DOES NOT notify Badger High School of the students who receive a physical.
