BAPTISM GUIDE & PERMISSION SLIP What baptism IS: 1. Baptism is an act of obedience to God. It’s an “assignment” from God as His child. (Acts 2:38) 2. Baptism is “publicly” identifying oneself with Jesus Christ as Lord and Savior. It is a way to show others that my life and way of thinking has changed because I have repented of my sin and received Jesus. (Matthew 10:32) 3. Baptism is a symbol, illustrating being dead to your old way of living and rising again to new life. (Romans 6:1-14) What baptism is NOT: 1. Baptism is not a way to be forgiven or accepted by God…that happens through personal repentance from sin and putting one’s faith in Jesus Christ as the perfect sacrifice for sin. (Ephesians 2:8-9) Would you like to be baptized as an act of obedience to God and as a way of publically saying you have turned from your sin and received Jesus Christ as your Lord and Savior?

My name is … I turned from my sin and received Jesus Christ as my Savior and Lord… Write two or three sentences on when you repented (turned from sin and a sinful way of living) and received Jesus Christ as your Lord and Savior. Example: I didn’t grow up in a Christian family. I went to church maybe once or twice. When I was 11 years old my mom asked me if I wanted to go to a Christian camp. So, in July of 1975, I went to Woodlake Ranch Christian Camp. I heard the gospel for the first time in my life. I repented of my sin and asked Jesus Christ to come into my life and wash my sins away when I was 11 years old at Woodlake Ranch Christian camp. My favorite Bible verse is… Write out one or two of your favorite Bible verses: Example: One of my favorite verses is John 1:12 “To all who receive Him (Jesus), to those who believe in His name, God gave them the right to become children of God.” Another one of my favorite verses is 1 John 1:9 “If we confess our sins, He is faithful and just and will forgive our sins and cleanse us from all unrighteousness.” I want to get baptized because… Example: I want to get baptized so that everyone knows that I believe Jesus Christ is my Lord and Savior. I hope that by taking this step that others will see my example and choose to follow God’s call to be baptized too. My life looks different now… Example: Since accepting Christ into my life, I’ve experienced so much joy. I have a desire to read God’s word and understand His plans for my life. I have a desire to bring Him glory in the decisions I make with my friends and it excites me to share with my friends what His great love means to me and how He desires to have a personal relationship with us all.
