Bayside Broncos Volunteer Commitment Form
As the player’s parent/guardian, I understand that Bayside Broncos depend heavily on Parent Volunteer Participation. Therefore, I understand that I am responsible for performing a minimum of FIFTEEN (15) hours of volunteer time to take place over the course of the current season and as assigned by my player’s Team Manager/Parent. By signing this form I acknowledge that I will participate in a Volunteer capacity and if I do not meet the FIFTEEN (15) hour requirement, I will forfeit the $250.00 deposit.

CHECK or CREDIT CARD NEEDED FOR VOLUNTEER DEPOSIT Volunteer Deposit - $250.00 per player – fully refunded after volunteer time has been fulfilled. Check or Credit Card will be held until the end of the season and will be returned after hours have been completed.
