Date: Sunday, February 28, 2016 Cost: $75.00 Location: NM State Soccer Athletic Complex (West of Aggie Memorial Football Stadium)
Participants: Open to all High School females Grades 9-12
9:30 am: Check-In 10:00 am – 11:00 am: Technical/Tactical Training 11:00 am – 12:00 pm: Small Sided Games 12:00 pm – 1:00 pm: Lunch (included) 1:00 pm – 2:00 pm: Chalk Talk 2:00 pm – 3:00 pm: Technical/Tactical Training 3:00 pm – 4:00 pm: Full Sided Games
** Goalkeeper Training Available **Q & A with NM State players and coaches **All participants will receive lunch as well as an NM State T-shirt! Registration, Parental Consent AND Medical Release Forms must be received by February 25th (Players will not be allowed to participate until all completed forms and payments are received)

Parental Consent Form The undersigned, being a parent or legal guardian of the child requesting camp admittance, does hereby affirm that the applicant is in good health and suffers from no illness, disabilities or condition that requires medication on a regular basis unless that condition is disclosed and approved. Furthermore, the undersigned has no knowledge of any reason the applicant cannot participate in vigorous physical activity. I understand that, as a condition of admittance as a camper, the undersigned on behalf of all parents and guardians and on behalf of the applicant, hereby releases the New Mexico State University, Freddy Delgado, Ben Parman and any camp employee from any and all liability from illness or injury, mental or physical, suffered by the camper during or related to camp, unless caused by willful act or gross negligence by the person or entity against whom the claim is made. Disclosure Per NCAA rules, all sport camps and clinics conducted by New Mexico State University are open to any and all entrants and enrollment is only limited based on age, grade level, gender, or number restrictions as specified by each camp. Release I/We, the undersigned, for ourselves, our heirs, executors and administration waive, release and forever discharge the New Mexico State University, the camp, its staff, officers, agents, employees, representatives, successors and assign of and from all rights and claims for damages, injury or loss to person or property which may be sustained during participation in camp activities or while at camp, whether or not damages, injury or loss is due to negligence. It shall be understood that participating campers are contracting with the employee and not New Mexico State University, and that the University and the State of New Mexico do not assume any contractual obligations for the conduct of the employee’s activity.
