CARL ALBERT ATHLETICS NECESSARY PAPERWORK FOR ALL ATHLETES 1. Rank One-this is our system for all paperwork for every athlete at Carl Albert. 2. Each athlete needs to have a physical (preferably from an MD, DO, ARN-P or PA). Physicals will be here at Carl Albert High School Thursday, May 17th. All proceeds go to the Athletic Training room supplies which are for ALL sports 3. Informed Parent Consent-this is to inform you, the parent, of all the risks, liability waivers, etc for your athlete. This form needs to be uploaded every year your athlete participates at Carl Albert. 4. OSSAA-this form is to make sure your athlete is in compliance for sports in the state of Oklahoma. PLEASE READ EACH QUESTION completely. If your athlete is filling out this form, please have him/her READ each question thoroughly. 5. Deadline for ALL paperwork is May 23, 2018. The goal is to get all paperwork posted by the end of this school year. This will ensure that all paperwork is updated before the end of the school year and will be ready for 2018-19 school year. 6. With the above mentioned date, this helps get all athletes ready for any summer participation. If paperwork comes later, it will be tended to when time allows. Please do your part in helping us. We have 496 athletes here at our school and getting them ready for participation takes time. Your help is greatly appreciated to ensure that all are ready to practice on DAY 1.
