OBJECTIVE: To provide intervention, case management and advocacy for pregnant women and parents struggling with substance use disorders in Circuit One. ORGANIZATIONAL RELATIONSHIPS: 1. Reports to:

Does not typically supervise any employees
3. Other: Works closely with the Director of Behavioral Health, other Case Managers, DCF, FFN, Healthy Start, Court System, Department of Corrections, Vocational Rehabilitation, and treatment providers QUALIFICATIONS:
Required o Bachelor’s degree in social services field or bachelor’s degree with an appropriate certification o Minimum of two (2) years of relevant experience with the Substance Use population and case management field o Knowledge of Trauma Informed Care, Motivational Interviewing, Strengths Based Planning and Brief Solution-Focused Interventions o Strong written and verbal communication skills, and ability to give instructions o Strong organization and time management skills o Strong critical thinking and decision making skills o Ability to use sound judgement when interpreting clients’ behavior and motivation o Ability to take initiative and work independently o Ability to work as a team player o Exhibit a broad understanding of cultural sensitivity o Knowledge of community resources o Valid Florida driver’s license and good driving record o Ability to transport clients o Proficiency in Word, Excel, and other computer related tasks
Preferred: o Master’s degree in Social Work o Non-profit and/or social services industry experience o Experience in case management for substance abusing population o Experience in provision of services to women and families
WORK ENVIRONMENT: Work is conducted primarily in a climate controlled setting, though employee does have to deal with seasonal weather in the course of day-to-day operations when meeting client needs and conducting home visits. Page | 1

Employee must be able to present to small and large groups for extended periods of time, remain in a stationary position for extended periods of time, move about the office or facility as needed, operate computer equipment, occasionally move heavy equipment and files, and operate in a high stress environment. Employee must be able to communicate well with management, staff, clients, community resources and stakeholders, while maintaining professional boundaries. Employee will be expected to provide his/her own transportation to and from work daily, and have the ability to transport clients and conduct relevant work related business requiring personal transportation. ESSENTIAL JOB FUNCTIONS: Works to provide substance use case management services to clients in Circuit One to meet the standards and objectives set forth by the WFIS program. ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
Provide case management services to eligible clients in Circuit One and efficiently manage a caseload of 15-35 clients Meet with clients as needed, no less than monthly, to support progress towards Intervention Plan goals Coordinate support groups which emphasize relapse prevention Provide effective services and respond quickly to signs and symptoms of relapse in clients Effectively manage rapidly changing priorities; balance office and field obligations; prioritize work demands; and manage stress constructively Advocate on behalf of clients as needed; advocate on behalf of clients in court as needs dictate Demonstrate understanding and sensitivity to a culturally diverse staff and clientele Maintain confidentiality in regards to client rights and HIPAA, CFR Title 42: Part 2, FS 397-501, and FIPA regulations by handling client and agency data in accordance with the confidentiality policies Communicate and work collaboratively with team members, attend team meetings, and communicate regularly with supervisors on program needs and issues; utilize individual/group supervision at least once per month and consult with supervisor as necessary Prepare routine agency reports, time sheets, medical record documentation, and community/court correspondence Adhere to all agency policies and procedures, including safety, ethics, and communication guidelines Attend all required agency trainings and meetings, provider agency staffing’s, treatment team meetings, graduations and mandatory meetings Continue professional development in the field of intervention, substance abuse, and mental health and complete required documentation Demonstrate an understanding of and participate in the agency Performance and Quality Improvement Plan Assist with special projects and other duties as assigned to drive the vision, fulfill the mission, and abide by the values of this agency
