SCHOOL PERFORMANCE REGISTRATION POLICIES AND PROCEDURES Registration Instructions Please complete the order form and retain a copy for your records. Mail your registration form to: School Performances, PlayhouseSquare, 1501 Euclid Avenue, Suite 200, Cleveland, OH 44115. Order forms may be faxed to 216-664-6069 (credit card and purchase orders only). Registration by phone is not accepted. Registration without payment cannot be accepted. You will receive confirmation of your order within two weeks of receipt of your order and deposit. If you have not received a confirmation, please re-submit your order and contact the Community Engagement & Education Hotline at 216-348-7909 to verify receipt of order. CHAPERONES For every 20 seats purchased, one chaperone may attend free of charge. Additional adult seats may be purchased at the student rate to accommodate the teacher or chaperone requirements of your school. PAYMENT INSTRUCTIONS A 20% non-refundable deposit is required at the time of your registration. All registrations must be paid in full 30 days prior to the performance. Payment can be made in the form of a check (school or personal), MasterCard, Visa, American Express or Discover.
If these options are not available, a school purchase order is a legally binding document which may be submitted in lieu of a deposit to secure your order. Invoices are sent to the school contact person listed on the registration form. Full payment for the balance must be submitted 30 days prior to your scheduled performance or your field trip may be cancelled. NO REFUNDS WILL BE ISSUED FOR CHANGES IN ENROLLMENT OR STUDENTS WHO ARE ABSENT THE DAY OF THE PERFORMANCE. REGISTRATION DATES Registration for all Community Engagement & Education paid events opens April 28, 2014. PlayhouseSquare accepts registration up to one week before an event, based on availability. Purchase orders cannot be accepted for late registrations. Full payment is required for all late registrations. We reserve the right to cancel any reservation that is not paid in full before the performance date. The amount of tickets in your order can be adjusted up to 30 days prior to your scheduled performance, subject to availability.

TICKET PRICES Pricing for Children’s Theater Series and International Children’s Theater Festival school performances is as follows: • Partners in Performance (PIP) School Districts • $5 per ticket • Non-PIP School Districts • $5 per ticket if ordered by September 30 • $6 per ticket if ordered after September 30 • Cleveland Metropolitan School District • $3 per ticket for all afternoon shows • $5 per ticket for morning shows • Small groups (less than 10 tickets) • $10 per ticket if ordered by September 30 • $11 per ticket if ordered after September 30

IMPORTANT: Due to the nature of the entertainment industry, all performances, dates and times listed in this catalog are subject to change without notice.
