A FIVE-STAR GIVER is an individual- man or woman, UMW member or non-UMW member, who gives to all five channels of giving: $40 to give someone special a Special Mission Recognition Pin $ 5 Pledge to Mission $ 5 Gift to Mission card $ 5 Gift in Memory card $ 5 World Thank Offering $60 Total amount handed into local treasurer as a one time gift. Your $60 offering must specify that you are giving to the above five channels and designate if possible to whom your pin will go, and that you are giving the $60 as a FIVE-STAR GIVER. You cannot simply give $60 to your Local treasurer - you must state that it is for FIVE-STAR GIVER and complete this form. Instructions to for Handling FIVE-STAR GIVER Awards This an award for Individuals only, not Circles or Local UMW Units 1. It is important to stress that this needs to be submitted all at one time, not spread out over the year but may be just a portion of regular giving. 2. The deadline to be recognized is August 15th of each year, to help the local treasurers complete all of the required reports on August 20. 3. Local Treasurers, when you receive these FIVE-STAR GIVER checks the amounts should be shown in your financial records as gifts to each of the five areas. For example, when you receive a check for $60.00, you will show it in your records as: $ 5.00 to Pledge $ 40.00 to SMR $ 5.00 to Gift to Mission $ 5.00 to Gift in Memory $ 5.00 to World Thank 4. The SMR request will be sent to your District Treasurer with a copy of the 5-star Giver form attached. Local Treasurer should keep a copy of form for their records as well. These monies would be added with all unit money and order Special Mission Recognition just like any other Unit orders. Please make sure that the FIVE-STAR GIVER understands that it may take up to 6 weeks to receive the SMR pin and card. SMR requests can be sent forward from a FIVE-STAR GIVER with “Honoree to be Determined” (blank without a name to honor) and can also be donated requesting no pin or card. In that case, the money is forwarded to National Office of United Methodist Women for SMR stating “no pin, no card” or can be requested as “no pin but receive a card”. 5. This program is open to non-UMW members. All church members should also be encouraged to participate. 6. The District Treasurer will need to receive from the Local treasurer with the name or names of the FIVE-STAR GIVERS by August 20th; so that an award pin can be sent prior to Annual District Meeting. 7. Local Treasurers will need to notify District or Conference Treasurer of Gift in Memory and/or Gift to Mission cards may be needed from their District Treasurer. 8. All the funds for FIVE-STAR GIVER will be sent to the District Treasurers with regular quarterly giving showing the totals for the Local unit in each area. The FIVE-STAR GIVER amounts should not be shown separately but the name of the giver in the Given by box of the Special Mission Recognition form. 9. The FIVE-STAR GIVER pins will be ordered from the Central Texas Conference Treasurer and the order should be received from District Treasurer by September 1st each year.
