To complete this application you must include the following: ¨ Completed CWU Cheerleading Application (this form) ¨ Completed New-Athlete Packet (including a current physical and copies of your insurance card) ¨ Signed CWU Cheer 2017-2018 Contract ¨ Tryout fee of $40, checks can be made out to CWU Foundations. Cash is also acceptable ¨ Proof of GPA 2.3 or above ¨ Copy of your acceptance letter to CWU or CWU connection card ¨ 2 letters of recommendation. **Use official recommendation form provided by the coach**

***Note: Upon making the team on April 22nd, each cheerleader will need to provide a check of $400 made out to CWU Foundations. This will ensure their spot and help get clothing orders processed in time*** I certify that the above information is true and accurate. I understand that if the information is incorrect or incomplete I will be excluded from the tryout process or removed from the team. I agree to follow the standards and policies set forth in the cheerleading guidelines and the CWU Cheerleading Contract and will cooperate fully with the cheerleading staff during the tryout process and throughout the 2017-2018 Season.
