WHAT IS NEEDED FOR SPONSORSHIP/ DONATIONS TO BE PROCESSED. • Any team receiving a sponsorship/donation from a company and or person MUST fill out the included Challenge FC Sponsorship form. • This completed form along with • a copy of the check and • the Chase deposit slip with coaches name, gender and age along with the word Sponsor/Donation wrote on the slip • need to be sent to the Challenge FC Regional Treasurer Dana Douglas • See examples
