Cheer Factory 2017-2018 Class Registration Agreement ________ I understand that I am registering my athlete _______________________, for the 2017-2018 session that runs June 1,2017 - May 30,2018. I also understand that if I decide to withdraw my athlete for any reason before the end of the session I must complete a WITHDRAWAL form the MONTH PRIOR. Provided I give the required 30 days notice, my tuition deposit will be applied to my final month of tuition. However, should I not provide Cheer Factory with the required 30 day notice, I will be responsible for paying a $50 early withdrawal fee IN ADDITION TO the last month’s tuition deposit, even if my athlete(s) chooses not to attend classes regularly. ________ I acknowledge that all tuition is due on the 1st of each month but will still be considered on time if paid NO LATER THAN THE 5th of each month. If my account is not paid by the 6th I understand that a $20 late fee will be assessed and all outstanding fees will be automatically charged to my debit/credit card on file. If tuition has not been paid by the 6th of the month, I understand that my athlete will not be able to participate in any classes/practices/competitions until our account is cleared up. _________ I understand that in the event my athlete will miss class for any reason, I need to call the gym office no later than the class start time to let Cheer Factory know of my athlete’s absence. If I call in as required, my athlete will be eligible to receive an open gym pass to makeup for the missed class. I also understand that my athlete is limited to 1 makeup pass per month. I understand that if my athlete misses more than 2 classes without notifying the gym office, my athlete may be dropped from their class.

NO ATHLETE WILL BE ALLOWED TO REGISTER WITH AN OUTSTANDING BALANCE. Accounts with a balance MUST meet with management before signing up for classes. CURRENT CF MEMBERS WITH DELINQUENT ACCOUNT HISTORY WILL BE REQUIRED TO PAY AN ADDITIONAL DEPOSIT PER ATHLETE UP FRONT TO REGISTER FOR A CLASS.

STANDARD BILLING PROCEDURES •  Fees will post to your CF account on the 1st of the month in which it's due. A late fee of $20 will be applied if not received by the 6th. •  All balances will be charged to the card on file on the 6th of each month. • If you prefer NOT to charge the card on file, you must bring in cash or check by the 5th of the month (or sooner should the gym be closed). Additional grace period will not be given if the 6th falls on a weekend. • All accounts are required to have a valid credit card on file at all times even if paying by cash or check monthly. It is your responsibility to fill out a new CC authorization form for lost/stolen/expired cards, billing address changes, etc. All applicable fees will be charged if the card on file is declined for ANY REASON. • For any autodraft account, if payment is returned for any reason you will have 24 hours to clear the account or a $20 late fee will be applied. In the event of any returned payment, any applicable NSF/processing fees will be charged to my account. • No account will be allowed to have fees roll over from month to month. • Athletes with a balance will not be allowed to participate in class until ALL fees are paid in full. • I understand that upon initial registration, I must pay a deposit equal to 1 month tuition, in addition to my first month’s tuition. In the event that my athlete withdraws from a class for any reason, as long as I provide the required written notice on time then the deposit will be applied to my last month of tuition. However, if my athlete is dropped from the class or withdrawals without the required written notice, the deposit will be forfeited. • Families with multiple athletes will receive a 20% discount off any additional sibling’s tuition. The 20% tuition discount will be applied to the lesser value of tuition when applicable. • If my athlete plans to drop from any Cheer Factory class, I must submit the required withdrawal notice to the gym office or email to [email protected] by the 1st of the month prior to the last month my athlete will attend.

PROGRAM RULES AND REGULATIONS FOR ATHLETES AND PARENTS

* If an athlete is injured, please notify Cheer Factory staff immediately. * No jewelry can be worn in the gym at any time. * Only water is allowed in the gym. * CF is not responsible for lost or stolen items so please use discretion when bringing valuables to the gym. Any unclaimed items will be donated. * If you are not a Cheer Factory athlete or staff member, do not enter the gym area. (Only exception is for the restrooms) * I will treat other athletes & staff with respect. I will treat and talk to others in the same manner I wish to be spoken to. * I understand if I threaten to quit or pull my child from a class, I may be dismissed from the program immediately. * I will not participate in negative gossip in any way. This includes on-line communication. Please remember that anything you ever say is a direct reflection on this organization and its members. * I will strive to maintain and improve upon the skills I have and set goals to acquire more skills. * I understand there must be perfection before progression. I will not attempt to learn advanced tumbling skills before perfecting the basics. * I will maintain good sportsmanship at all times when representing Cheer Factory. Abusive behavior, lying, and/or any other form of negative behavior are grounds for removal from this program. * I understand that the parent viewing areas should be a positive place, and I may be prohibited from using the parent viewing area if I cannot maintain a positive attitude or keep comments to myself. Furthermore, I understand that the parent viewing area is not my personal workshop, living room, dining room, party room or day care. I will respect others and use it for the sole purpose of viewing my child. I have read and understand the 2017-2018 Registration Agreement in its entirety and agree to abide by all guidelines set forth by Cheer Factory. I understand that failure to adhere to any of these policies will result in removal from the program with no refund.
