Joel Breiner and Leon Mullinax, ECI Construction, Inc. Marty Silman Final Punch List October 2, 2015
Please find below a consolidated list of remaining items that will be required for closeout of the City Walk project and acceptance of the new portion of Water Street. The list has been broken out into four sections, Water Street, City Walk, CCTV and Final Items. Once the final CCTV items are completed, the City will re-inspect the underground work via camera inspection. It is the contractor’s responsibility to notify the City when all items are corrected. All items need to be resolved prior to acceptance of Water Street.
Water Street Punch List 1. Install street lights on E Water Street. (Not Complete.) 2. Replace any dead or distressed trees. (There are three trees that have been flagged with pink ribbon. One is completely dead; the other two are showing obvious signs of distress.) 3. There are a couple depressed areas at the edge of the asphalt trail towards Meade Ave. These need to be fixed. (There is still a depressed area near the utility pole on the western end of the trail.) 4. Convert the sediment basin to the permanent SWM facility. (As-built of the pond shows a 7” orifice that is not in the approve design plans. Plug or provide analysis to verify need.)
City Walk Punch List 5. All site related items have been completed.
CCTV Inspection Punch List 6. As identified in the report by Racey Engineering, the City accepts and approves the method of correction for the following structures: a. B6 (During the initial repair efforts, this structure was found to be damaged beyond repair and will need to be replaced. This was outlined in an email on 7/22/15.) 7. The pipe penetration in the following structures needs to be repaired. The existing concrete around the pipe should be removed and new mortar applied. The opening of the pipe should not be obstructed. b. B10.1 (This pipe still has debris in it.)
Final Items Punch List 8. As a result of the repairs needed along Water Street since the final paving was done, additional paving work is needed to address the numerous pavement patches. From approximately station 11+00 to station 15+00 and from City Walk Way to Carlton Road should receive a 2” mill and overlay. Any striping that is removed or damaged as a result of this paving should be replaced. Please schedule an on-site meeting with City Staff to confirm limits prior to paving.
9. Per Section 5.02 of the City’s landscape specifications, which were provided with the Streetscape and Parking Improvement Plans, the contractor shall guarantee all materials and labor performed for a period of one year from the date of final acceptance. Please provide a formal letter stating acknowledgement and commitment to this requirement. 10. Finalize as-built plans to include the light locations after they are installed and reflect the completed stormwater facility. 11. Finalize plat to show easements needed for the lights/electric meter and maintain 46.5’ ROW width on the western end of Water Street near 10th. Please provide a schedule for anticipated completion of the above items so that staff may schedule for the re-inspection of repaired storm infrastructure and place the street acceptance on the appropriate City Council agenda. Upon your review of the above information, should you have questions or required additional information, please do not hesitate to contact me at 434-970-3991 or
