How to Enroll

If you have a medical condition, it’s important to understand how it affects you and how to follow your doctor’s treatment plan. The Condition Support program can help you do just that. When you enroll in the program, a condition support manager works with you to manage medical issues such as asthma, diabetes, hypertension and heart disease. Contact your medical carrier to see if the program has services for your condition. You may even qualify for discounts on the prescription medication you take for your condition (p. 2).

Eligibility Requirements You’re eligible for the Condition Support program if you’re enrolled in the HSA, HSA Plus or HRA Core medical plan. Family members covered by your medical plan are eligible too. There’s no extra cost for you and, in some cases, you may be able to earn Vitality Points™ by participating in the program (see below).

If you’re not covered by the HSA, HSA Plus or HRA Core plan, contact your medical plan carrier to see if a similar program is available to you.

Earn Vitality Points You can earn 300 Vitality Points every quarter you participate in the Condition Support program. HSA, HSA Plus and HRA Core Plan Members The Vitality Points you earn show up automatically in your Vitality account as long as you enroll and complete the program. It can take up to 90 days for the points to show up on your Vitality Points Statement.

Members of Other Medical Plans If your medical plan offers a similar program, submit the Disease Management Engagement Form and proof you participate to Vitality to earn points. Download the form at Vitality > Forms and Waivers > Disease Management.

Save on Your Prescription Medications

Discounts for HRA Core Plan Members If you’re in the HRA Core medical plan, you may be able to get discounts on medications for an on-going condition. Talk with your condition support manager to find out if you qualify for the Condition Support Medication Discount. You need to stay in contact and work with your condition support manager to get the discount. Once enrolled in the program, you can find your active dates under the Incentives tab on UPoint. Here’s What You Pay • Nothing for generic drugs (instead of 20%). • 10% coinsurance for preferred brand name drugs (instead of 20%).* • 20% coinsurance for non-preferred brand name drugs (instead of 40%).* Even better, you don’t have to meet your deductible to get your discount. Your health reimbursement account is used only to help pay your share of coinsurance. * Step therapy program rules are applied before any discounts. Preventive Prescription Drug Discounts for HSA and HSA Plus Plan Members Good news if you’re an HSA or HSA Plus plan member. You don’t have to participate in the Condition Support program to receive discounts on your medications. You can get the same discount as HRA Core plan members if your medication is on the HSA Preventive Therapy Drug List.
