1. Tuition Payment Plan Selection: Please CIRCLE one of the payment plans below. Plan A: Full tuition balance, less the deposit, payable by June 1, 2016. There is a $500 non-refundable deposit ($100 non-refundable registration fee and $400 non-refundable tuition deposit), per child, required upon registration. Early bird registration ends March 15, 2016. If registration is received after March 15, 2016, the required non-refundable deposit will increase to $650 ($250 non-refundable registration fee and $400 non-refundable tuition deposit), per child.

Only accounts that are paid in full or on a payment plan schedule are eligible for early bird rates.
•
Registration will not be accepted until all prior CAI financial obligations are paid in full, or are on an existing Payment Plan Schedule.
•
Families enrolling multiple children receive a 10% discount on tuition only, for the younger siblings enrolled.
•
If your child begins school after the first day of the school year, payment for the full month in which he/she begins will be due immediately.
•
A $100 administrative fee will be applied for each change made after August 1 to your child’s initial program selection.
•
All applications are subject to the approval of the Director.
•
Tuition payments may only be applied to the current year and are not transferable.
•
Withdrawal: Deposits, registration fees, and tuition payments are not refundable in the event of withdrawal. However, for those families who select Payment Plan A (full payment before June 1, 2016), refunds will be provided as follows: Withdrawal before August 1: Refund of 50% of tuition Withdrawal before September 1: Refund of 25% of tuition
Registration will not be accepted without this form, fully completed with Tuition Payment Plan Selection, nonrefundable deposit payment, and voided check or credit card information (unless choosing Plan A.) I have enclosed a $500 non-refundable deposit, per child, Tuition Payment Plan selection, E-check/Credit Card authorization and application.
