Cougar Honors Nomination Form
We honor Catholic Central High School athletes and athletic staff whose performance provided exceptional contribution to the Cougar athletic program either by theirr performance on athletic teams or by their exceptional effort on be behalf of the athletic program. Honorees are recognized with a ceremony and plaque laque in their honor at Cougar SStadium.
Eligibility: Student-athletes athletes must have graduated at least ten yyears ears prior to nomination and earned multiple athletic honors beyond varsity letters. Coaches/Admin and Support must have completed five years of service to Catholic Central and departed/retired at least five years prior tto o nomination. Additional Honors Program ogram details and nomination forms are available at www.grcatholiccentral.org/athletics .
Complete this form and forward to [email protected] no later than March 15th of each year for consideration at Selection Com Committee meetings in the spring. Nominations remain eligible for three years.
