Creeks Athletic Association Background Check Procedures . *Each league will be billed for their background checks completed
Step 1 Complete background check application. The background check application can be found on the CAA website (caaleague.org). In the upper right hand corner of the application, indicate volunteer position (ex. coach, head coach or team mom)
Step 2 To complete application you will need: * driver's license number or state id number
Step 3 Attach a 2X3 photograph/picture to the background application. If you cannot provide a photograph/picture, please stop by Mills Field location and your picture will be taken. See below address hours of operation
Step 4 Once application is completed submit to league organizers. Then the league organizer of each sport will hand in all applications, in batches, to the CAA Administrative Assistant, at the Mills Field Location.
Applications can be mailed or dropped off at: 1721 Race Track Rd Saint Johns, FL 32259 (Mills Field) Office hours are Monday, Wednesday and Friday from 9am-2pm Tuesday and Thursday from 5pm-8pm
Step 5 Background application will be processed by BlueLine Services (a 3rd party background screening company). Results within 24 hours. Step 6 Once results have been reviewed. Approval will be given to volunteers for each sport and badges will be printed. Step 7 Volunteers will then be issued a form of ID which can be picked up at the Mills Field location. League representatives can pick up badges for their sports and deliver them directly to volunteers.
Step 8 Each league will be billed directly for the cost of badging the volunteers for their sport at a cost of $10 per applicant. A complet list of all the background submissions will be submitted to each league for reimbursement to CAA.
