CYP Rewards Tracking Form Instructions Thank you for sharing your time and talents with your CYP! Please use this form to document the hours you spend volunteering in the CYP (with those activities designated as "hourly opportunities"). Your child's CYP Professional can answer any questions you have about the CYP Rewards Program and help you get started tracking your points. Please follow the steps below to track your CYP Family Reward points. Steps for tracking your CYP Reward points: *Write your name and your child's name at the top of the form. Your child's classroom or front desk has a CYP Rewards Binder where you can store your form between volunteer visits. *When you volunteer, write the date, a brief description of the activity, start and stop times, and the total number of hours (including half hours) you spent participating. *Have a CYP Professional verify your time by signing the form at the end of that line. *Continue entering volunteer activities. Once you reach 10 or more points, take the form to the CYP Operations Clerk at the front desk to redeem your points for a $30 discount off of your next Military Payday child care fees. Steps for Redeeming your points (once you have earned 10 points): *Once you have earned 10 points, take the form to the Operations Clerk to redeem your points and receive a $30 discount on your next child care fees. *The CYP Director must verify and initial that the full 10 points have been earned. *The Operations Clerk fills in next Military Payday Date (MPD) when the discount will be applied and initials the form once the auto pay discount has been set up into the system. *The Operations Clerk will transfer any leftover points onto a new form. You may hold onto the form or store it in the CYP Rewards Binder for future volunteer tracking. *Once the Operations Clerk scans the completed form in the system, you may choose to keep the form or the CYP will shred it.
