How To – Documenting EKG Reports This How To will show where to document EKG interpretation in the Note using Dragon, free text, or the EKG note form and what the interpretation should include. 1.
Documenting using Dragon or free text: In the Result/Data section of the note, document EKG interpretation using the
EKG Report text box.
2. Documenting using EKG Report note form: In the Result/Data section of the note, document EKG interpretation using the
EKG Report note form.
NOTE: The documentation of the interpretation should include: o Rhythm o Rate o Axis, o Intervals o ST segment change o Comparison to a prior EKG o summary of clinical condition 10.27.15
