College Visit Authorization Form Eisenhower High School has implemented this procedure for students taking college visits: 1. A Senior may take five (5) college visits per semester. 2. A Junior may take two (2) college visits second semester. 3. The absence is only waived if: ü the parent/guardian has excused the absence prior to the visit. ü the student has made a visit reservation with the college at least a week in advance of the visit date. ü this form is obtained by the student before the visit. ü the student stays for the full visit.
