Fill out a Health History and Consent Form available at your child’s school office. Return form to school office.
The completed form will be faxed to our dental office by the school nurse.
We will contact you to schedule the dental appointment.
DENTAL VAN A Health History and Consent Form must be submitted prior to scheduling – ask for one at your school office.
We are proud to announce the arrival of the El Dorado Smiles Dental Van. The El Dorado Smiles Dental Van will provide comprehensive dental care to children, in collaboration with El Dorado Schools.
Transportation to and from the Dental Van is not available.
