ENA Release Form 1. I hereby certify that my child is in normal health and capable of safe participation in ENA sports programs. I assume all risk(s) and hazards incidental to the conduct of this program. I hereby authorize ENA to obtain medical treatment for my child in the event the parent(s) cannot be reached 2. I support the ENA philosophy based on character development, participation, fun, skill development, teamwork, fair play, and growth in mind body and spirit.( our mission statement and goals can be found on our website under “About ENA”) 3. I will read and follow the ENA code of conduct which can be located on our website under the resources tab
