Q2R23 – March 2016 • Modifications – None • UT - None • Visual Scope − Core Spray Piping – 36 Welds, all NRI − Core Spray Sparger – 2 Lower drain plugs with NRI (But with missing tack welds) − Interior Attachments – 4 WSL, 5 GRSBA, 10 SSH brackets with NRI, except: • On-going wear to Dryer WSL − CASS Components – 7 items on JP and CRD GT (NRC License Renewal Commitment), all NRI − 2 Top Guide Aligner Assemblies/Pins with NRI − Top Guide Rim Weld – 20 Cell Locations with NRI

Q2R23 – March 2016 • Visual Scope − 7 FW Sparger End Brackets • On-going minor wear, with slight additional wear this exam − 14 Dry Tubes per SIL 409-R5 • 3-SRM, 3-IRM, 2-LPRM using EVT-1 2-sided due to age for cracking • 1-SRM, 5-IRM using VT-3 1-sided to for baseline on top plunger engagement • No cracking identified • Found 8 SRM/IRM with some loss of plunger engagement • Found 5 SRM/IRM with minimal plunger engagement • Performed 1-cycle evaluation • Repair next outage (replace Dry Tube or install Plunger Extension-Patented) 3
Q2R23 – March 2016 • Visual Scope − Shroud Repair Hardware at 4 Locations with – RI • Spring Ratchet not fully engaged, follow-up known issue-no changes noted − Jet Pumps • 42 High and 30 Medium Priority Welds with NRI • 2 JP Swing Gate/Main Wedge and rod with previous RI - Known minor wear indications on the rod - Follow up exams show no changes • 3 JP Aux Wedges with NRI • 8 Swing Gate Retainer Cup Tack welds - 2 as a follow-up for bad/missing welds with no changes noted • 1 Slip Joint repair clamp with previous minor wear – no changes • First weld draw bead identified on a riser pipe 4
