Are You a New Federation? Welcome • You must submit along with your application the complete applications of all member organizations. • Newly created Federations operating for less than a year (as determined from the date of the IRS determination letter to the closing date for the CFC application period) are not required to submit audited financial statements.

A complete application for yourself; For each member that is new to the Federation (regardless of whether it participated as an independent organization or member of another federation in 2015 CFC) and For each member denied participation in 2015.

Local Federation Eligibility 5 CFR §950.303 • 15 or more charitable organizations, in addition to the federation itself, that meet the eligibility criteria of §950.292, §950.023, and §950.204 • Certify all member organizations seeking participation in the CFC are qualified for inclusion on the Local List • Meet eligibility requirements and public accountability requirements.

2016 Required Application Attachments • Attachment A- Federation Membership Listing. The Federation Spreadsheet is where you will provide the listing of your members. The physical mailing address of the federation and each member organization applying for participation in the CFC must also be included. • Attachment B- IRS Determination Letters that verifies the taxexempt status of the Federation and each of its member organizations as well as DBA (Doing Business As) documentation, if applicable. • This also covers includes the Catholic directories, Bona-fide Chapter letters, National organizations tax exemptions and etc.

2016 Required Application Attachments • Attachment C- A copy of the Auditor’s report and the Federation’s complete audited annual financial statements. • You must account for the funds on an accrual basis in accordance with Generally Accepted Accounting Principles (GAAP) and Generally Accepted Auditing Standards (GAAS). • The audited financial statements must cover the fiscal period ending not more than 18 months prior to January 2016 (ending on or after June 30, 2014). • The audited financial statements must verify the federation is honoring designations made to each member organization by distributing a proportionate share of receipts based on donor designations to each member.

2016 Required Application Attachments • Attachment D- Complete, Signed IRS Form 990 for a period ended not more than 18 months prior to January 2016. • The IRS Form 990 and audited financial statements must cover the same fiscal period. • Attachment E- A complete listing of the Federation’s Board of Directors. • The beginning and end dates of each individual’s current term of office. • List the Board Meeting dates and location for the previous calendar year (2015) • Attachment F- Annual Report (18 months prior to January).

Common Application Errors Look out for the following issues: • Out of Area • Missing check marks in the boxes. You must certify at least one response for each question. • Hours of operation not listed • Incorrect addresses provided • The AFR certified on the application doesn’t match what shows up on the on-line calculation. • Missing Attachments • 990 Issues • • • •

2016 Local Federation Application •
The application will go live on December 18, 2015 and it will be completed on-line via the Nexus Charity Application ($30 Federation Application Fee) at https://www.charitynexus.org/_apply.

Federation Sampling • LFCC and OPM reserves the right to sample 20 to 100 percent of federation applications
• If you are the selected Federation we will notify you by February 1, 2016 and you will need to have that sampling in our office on or before February 8, 2016 at 5pm

Notifications • Denial Notifications will be distributed on April 12, 2016 • Approval Notifications will be April 19, 2016 • Appeals Process- Appeals must be received by the LAC on or before April 26, 2016, the LAC will review them on April 28, 2016 and decisions will be delivered certified mail by May 2, 2016 • If you are denied by the LAC you can submit an appeal to OPM by May 16, 2016

Notifications (continued) • Requests for charity withdrawals will only be accepted with the submission of a certified letter from an authorized charity and federation representative • Responsiveness to LFCC and OPM request Failure to demonstrate satisfactory corrective action or to respond to a request by the LFCC or OPM Director for information within 10 business days of the date of the request may result in a determination that the local federation will not be included in the Local part of the Charity List

OPM’s Validation • All charities applying to CFCNCA are submitted to OPM annually for IRS verification • LFCC and OPM approves local federations and member charities that meet all of the applicable requirements • OPM checks for charities in IRS Master Business File by EIN number • Charities not validated by OPM are not included in the CFCNCA catalog

Please keep in mind… •
The decision of the Office of CFC Operations is final
•
Submission of requested documents is not a guarantee that OPM will accept your submission
•
Collecting as much documentation as possible with the applications will save time when the deadline approaches
•
Any charity statement that uses special features, or exceeds 25 words will be edited by the LFCC
•
Organizations will be listed by their legal IRS recognized name as it appears on the IRS determination letter only unless the appropriate legal documentation is provided
•
Completed Federation spreadsheet checklist is a requirement.
