In consideration of being allowed to participate in any way in programs, related events and activities of the Midland Civic Arena, I, the undersigned, acknowledge, appreciate, and agree that: The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, I knowingly and freely assume all such risks, , both known and unknown of my participation in Midland Civic Arena activities, even if arising from the negligence of the releases or others, and assume full responsibility for my participation; and, I willingly agree to comply with the stated and customary terms and conditions for participation in Midland Civic Arena activities. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless the Midland Civic Arena, the City of Midland, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event (“Releasees”), with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the releases or otherwise, to the fullest extent permitted by law. I consent and authorize the City of Midland to publish the name and photograph(s) of my children or ward(s) whose name(s) are listed above, for any lawful purpose. I understand that I will not be compensated for the use of the name(s) and photograph(s) of my children, and will hold the City of Midland harmless in its use thereof. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. __________________________________________________________________

Season Consists of : 1 Divisional Clinic, 2 Team Practices, 6 Games, NFL Reversible Jersey
Teams Teams will be formed by a draft. All players registered will be placed on a team.
Games Will consist of a 10 minute warm-up and two - 20 minute halves.
Forms Needed Concussion Form Registration Form w/ signed waiver
Equipment Mouthguard required (available in pro shop) No Metal cleats allowed on turf Flags will be provided
Tyke Division - K-2nd Grade Prep Division - 3rd & 4th Grade Junior Division - 5th & 6th Grade May modify divisions due to low participation
