We are pleased to offer this service as part of your wellness benefit. Presenting an Express Scripts Pharmacy Benefit card from the State covers the cost of immunizations with no copay. Other insurances will cover certain immunizations but will be considered on a case by case basis.
Bring your Express Scripts Pharmacy card:
Flu, Pneumonia, Hepatitis A and B, Tetanus/Pertussis (whooping cough), Meningitis. More information regarding all of these vaccines is available on the Center for Disease Control website at www.cdc.gov/vaccines The Centers for Disease Control recommends everyone get an influenza vaccination every year as soon as available. Whooping Cough cases are on the rise and the Department of Health recommends getting vaccinated. These, as well as other immunizations are available with no co-payment with your Express Scripts prescription drug insurance.
Without insurance Flu shots are $35.00. We can also bill Medicare and some private insurance plans.
The required registration form is attached to this message and is also available at southwestpharmacy.com. Blank forms will be available at the clinic and walk-ins are welcome.
