Dear Parent/Guardian, Loveland Youth Baseball has a policy regarding the financial assistance (scholarship program) for our youth players. If your child is on the Federal Lunch Program, they are eligible for financial assistance. Procedure: 1. Please take the attached form to your child’s school and have a school official sign the form verifying your child is on the Federal Lunch Program. This information is kept completely confidential. Coaches are not aware of players who have applied for and/or are on a scholarship. 2. Return the completed form to: Loveland Youth Baseball P.O. Box 201 Loveland, OH 45140 3. Once your request is approved you will be contacted via email with a registration discount code. 4. You must use the registration discount code to register your child at www.lybo.org or they will not be placed in the draft for a team assignment. 5.
