I/We, the parent/guardian of the above named candidate for a position in the M.O.T. Youth Football & Cheerleading League, hereby give approval for him/her to participate in any and all football and cheerleading activities. I/We are aware that football and cheerleading are activities in which the risk of injury is high. These activities in general could lead to serious injury, including partial or total paralysis, even death. I/We have discussed this with our child and among ourselves. Despite this understanding of the possibility of serious or catastrophic injury or death and the risks involved, we still consent to the participation in this activity by our son/daughter. I/We represent to you that, to the best of our knowledge and belief, our son/daughter has no physical, medical, or mental disability or other limitation that would restrict his/her ability to fully participate in this activity as described and explained to us. I/We agree to furnish a Statement of Physical Condition signed by a physician, which states that my/our child is physically fit to participate in contact sports. The statement must be dated after January 1, 2013 and furnished the first week of practice. Equipment will not be provided without this statement. I/We accept full responsibility for any team uniform and equipment that my/our child is allowed to use and will pay to replace any and all uniform and equipment lost or destroyed except through normal wear and tear. The replacement value will be established upon notification of loss. I/We will not alter the uniform and equipment in any way as to render the manufacturer's warranties null and void. I/We will return the uniform and equipment washed and cleaned promptly at the end of the season as requested by the team. I/We agree to furnish an original and certified true copy with raised (not photocopy) of my/our child's birth certificate during the first week of practice. I/We agree to allow the M.O.T. Youth Football & Cheerleading League, Inc. to release photographs of my/our child for publicity in newspapers and on our web site. I/We agree to, and by the signing of this agreement, release the coaches, assistant coaches, volunteers, Board Members of the M.O.T. Youth Football & Cheerleading League from any claim of negligence by ourselves, our son/daughter, our heirs, executors and assigns, from any liability arising from claims for damages for injury to our son/daughter and any claims for loss of or damage to his/her property which may arise out of his/her participation in the M.O.T. Youth Football & Cheerleading League for the 2013 season.

Parent / Player Code Of Conduct (Please Read) The essential elements of character building and ethics in sports are embodied in the concept of sportsmanship and six core principles: Trustworthiness, Respect, Responsibility, Fairness, Caring, Good Citizenship. The highest potential of sports is achieved when competition reflects these six pillars of character. My Child/Children and I/We Agree: 1. I will not force my children to participate in sports. 2. I will remember that children participate to have fun and that the game is for the youth, not adults. 3. I will inform the coach of any disability or ailment that may affect the safety of my children or the safety of others. 4. I will learn the rules of the game and the policies of the MOT Youth Football and Cheerleading League. 5. I and my guests will be a positive role model for my children and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all players, coaches, referees, League Officials, and spectators at every game, practice, or other sporting event. 6. I, my guests, and my children will not engage in any kind of unsportsmanlike conduct with any League Official, coach, player, parent, or referee such as booing, taunting, refusing to shake hands, or using profane language or gestures. Includes all practices and games. 7. I will not encourage any behaviors or practices that would endanger the health and well-being of the athletes. 8. I will teach my children to play by the rules and to resolve conflicts without resorting to hostility or violence. 9. I will demand that my children treat other players, coaches, League Officials, referees, and spectators with respect regardless of race, creed, color, sex, or ability. 10. I will teach my children that doing one’s best is more important than winning, so that my children will never feel defeated by the outcome of a game or his/her performance. 11. I will praise my children for competing fairly and trying hard, and make my children feel like winners every time. 12. I will never ridicule or yell at my children or other participants for making a mistake or losing a competition. 13. I will emphasize skill development at practices and how they benefit my children over winning. I will also de-emphasize games and competition in the lower age groups. 14. I will promote the emotional and physical well-being of the athletes ahead of any personal desire I may have for my children to win. 15. My children and I will respect the League Officials and umpires and their authority during games and will never question, argue with, or confront coaches at the game field, and will take time to speak with coaches at an agreed upon time and place. 16. I will demand a sports environment for my children that is free from drugs, tobacco, and alcohol, and I will refrain from their use at all sports events. 17. I will refrain from coaching my children or other players during games and practices, unless I am one of the official coaches of the team. 18. All vehicles must be parked in the parking lot. No vehicles shall drive onto the grass area unless approved by the MOTYF&CL Board. 19. All dogs that are brought up to the MOT YFCL Complex must be on a leash. 20. Failure to comply with the above listed statements may result in suspension or other disciplinary action as deemed appropriate by the MOTYF&CL Executive Board.
